rename TOOL_BUILD_DIR to TOOLCHAIN_DIR
...
To reflect the new output directory hierachy rename the Makefile variable
TOOL_BUILD_DIR to TOOLCHAIN_DIR.

...
The host stuff seems pretty broken though as we don't build a host version
of liblzo.
